How do I enroll in MyMy record patient portal?

My Record Patient Portal gives you the flexibility to access your health information on your time and between visits to Whitman Hospital & Medical Clinics. Enroll at Whitman Hospital through Patient Registration (Hours: M-F 7:30am to 5pm, Weekend 9am to 5pm). Use the Current Members button. To self-enroll, use the Enroll Here button.

How to access Illinois nursing staffing information?

In accordance with the Illinois Report Card Act [210 ILCS 86], the public has the right to access nurse staffing information. Requests for staffing information can be obtained by calling the Director of Nursing (DON) at 327-2672 Monday through Friday 8:00 a.m.-4:30 p.m.

Does WCH use SSM?

WCH is excited to announce our use of SSM MyChart. This will allow our patients online access to their health information, communicate with their providers, and request prescription refills. Coming soon, patients will also be able to pay their hospital bills.

Washington County Nursing Home is an 88 bed long-term care facility located in Chatom, Alabama. Our primary focus has always been to provide quality long-term care to the people of Washington County and the surrounding area.

Hours

Our clinic on the Lincoln Square is open 8:00 a.m. – 4:30 p.m., Monday through Thursday, and 8:00 a.m. – 12:00 p.m. on Friday. The school-based clinic located on the Lincoln Middle School Campus is open 7:30 a.m. – 3:30 p.m., Monday through Wednesday, and 7:30 a.m. – 1:00 p.m. on Friday. Closed on Thursdays.

Holidays

We are closed New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving (Thursday and Friday), Christmas Eve and Christmas Day.

Inclement Weather

Please check local television stations and Washington Regional social media for announcements regarding clinic closures due to inclement weather. Patients should call the clinic before coming in during inclement weather.

Emergencies

In case of an emergency, please call 911 or go to your nearest hospital emergency room.

Prescription Refill

Please call your pharmacy 2 - 3 days prior to running out of medication so the pharmacy can fax a refill request to the doctor if needed. Requests received after 4 p.m. will be processed within the next two business days.

Medical Records

A medical request signed by the patient is required to send or release records to anyone. If you are requesting a copy of your records, there will be an additional fee.

Financial Requirements

We accept most major insurances. Your insurance card will be requested at each visit. Copays, deductibles and/or co-insurance must be paid at time of service. We accept cash, checks and credit/debit cards (Visa, MasterCard and Discover).
