How do I submit a request to Premier Family Physicians?
Patients and third party requestors are now able to submit a request online using Healthmark's secure website. Simply click here to create an account and submit your request. This service is currently only for requestors that want Premier Family Physicians to release information.

For a more convenient way to interact with our office please download the Premier Family Physicians App which is free in both the Apple App Store and Google Play. The Patient Portal can be accessed using any computer, tablet or smartphone with an internet connection. The patient portal is not to be used in emergencies. If you require urgent or immediate medical care, call 911.
