We are excited to present Premier’s online Patient Portal as another step towards creating more efficient and effective communication between you and our office. The Patient Portal is part of your personal electronic health record, and allows patients to conveniently access their health information from the comfort of their home or office.

How do I submit a request to Premier Family Physicians?

Patients and third party requestors are now able to submit a request online using Healthmark's secure website. Simply click here to create an account and submit your request. This service is currently only for requestors that want Premier Family Physicians to release information.

How can the Premier Family Physicians app help you?

For a more convenient way to interact with our office please download the Premier Family Physicians App which is free in both the Apple App Store and Google Play. Easily complete patient forms, upload insurance information, and connect with your preferred care provider via Virtual Visits to receive the care you need anywhere, anytime.

What can I do with the new patient portal?

Now you can! Our new electronic patient portal will allow you to do all of the above-named tasks and more. You may review your health history, obtain labs, see immunization history and see office visit information at your own convenience and from the comfort of your own home.

How do I Sign up for the Patient Portal?

Signing up for the Patient Portal couldn’t be easier: all you need is a valid e-mail address and an internet connection! To enroll, use the form on this page or stop by the front desk and speak to any of our staff . They will enter your e-mail address into our system and provide you a secure ID along with instructions on how to finish the enrollment process at home or on your smartphone.

If you have questions about self-enrollment, call Cerner Care at (877) 621-8014. If you have questions about the results on your Patient Portal, you need to contact your physician or health care provider.

Can you use a patient portal in an emergency?

The Patient Portal can be accessed using any computer, tablet or smartphone with an internet connection. The patient portal is not to be used in emergencies. If you require urgent or immediate medical care, call 911.

Is the Patient Portal a complete medical record?

The Patient Portal is not a complete medical record. If you need to obtain your medical record, please call (432) 221-1600.
