What is PHI in medical terms?
An individual has a right to receive a copy of his/her Protected Health Information (PHI) in the form, format and manner requested, if readily producible in that way, or as otherwise agreed to by the individual.
Can you get medical records from Manchester Memorial Hospital?
A valid ID is required. Authorized legal representatives must provide appropriate documentation for verification. Please note that if you need medical records from a specific doctor outside of ECHN’s Medical Group providers, you will need to contact that doctor’s office directly.
What is the Patient Portal at Memorial Hospital?
The Patient Portal program gives you the flexibility to access your health information and other resources on your time and between visits to Memorial Hospital. The portal is available over the internet, which means that you can use it from virtually anywhere. You can also use the patient portal to access information for family members and individuals for whom you provide care if given permission.

Is the Patient Portal secure?
The Patient Portal is completely secure, so you can be confident that your private information is protected. Only you – or an authorized family member – can access your account. The patient portal protects access to this sensitive information through a number of security measures, including URL rewriting, encryption and customer-defined timeouts.
Can you use the patient portal for Memorial?
You can also use the patient portal to access information for family members and individuals for whom you provide care if given permission. The internet program works in conjunction with Memorial’s Cerner computer system, allowing patients the ability, through a secure password, to view their own private information.
