You are encouraged to contact our office via telephone if you have any questions or needs. The Grove Medical physicians will decide which medical topics are appropriate for online communications and with whom we communicate with online. The Patient Portal allows better communication with your physician's office by providing convenient 24 x 7 access from the comfort and privacy of your own home or office. Communicate with practice, securely and efficiently.

How do I sign up for Family Medical Center’s online account?

This is a FREE service provided by Family Medical Center. In order to create your account, you’ll need an invitation code (usually the last four digits of your social security number). Please contact our office to enroll and receive your invitation email.

Can other people access my medical records?

Can you access the patient portal from public computers?

Jacob Hostetler, MD

Mary "Lainie" Littlefield is a Nurse Practitioner with over 40 years of experience in the nursing field. She received Associate of Nursing degree from Pima Community College, Tucson, AZ in 1975.

Shealyn Berry, DNP, APRN-CNP

Shealyn Berry is a Doctoral prepared Family Nurse Practitioner with eight years of experience in the nursing field. She received an Associate of Nursing degree from Northwest Arkansas Community College, Bentonville, AR in 2011. She received her Bachelors of Nursing from Northeastern State University, Muskogee, OK in 2013.
