The Northwest Florida Community Hospital Patient Portal is an online health management tool that allows you to view clinical data from your medical record. To access NFCH clinic patient portal click the link below. The MyCareCorner patient portal provides patients with access to their electronic health record.

What is the Florida Health Connect patient portal?

Florida Health Connect is a patient portal that is free to everyone receiving services at any local county health department. The portal allows you to more actively engage with your care team at a time that works best for you. Tired of waiting on a call back? Florida Health Connect will make talking to your care team easier.

How do I request an invite to the patient portal?

If you are currently a patient with our clinic, simply request your secure email invitation to our new Patient Portal via MyHealthRecord by clicking the “Request Portal Invite” button above. Once you request an invite, you will receive an email within 3 business days to register for the Portal.

Patient requests

In order to verify your identification and validate your authorization, we require a legible copy of a valid photo I.D. (e.g., driver’s license, military I.D. or state I.D.).

Urgent requests, records for your physician

For immediate continuity of care, your healthcare provider can request records.

Non-patient requests (insurance, attorney, disability and third party requests)

Requests should be sent from your insurance company, attorney, or Disability Determination Service and mailed to the address below. We cannot accept faxes and email requests. For assistance, call (888) 616-5721.

Patients 13-17 may have their own portal account. Patients age 18 and older may grant proxy access to their portal account through an approved Personal Representative Authorization Form, which may be done at any Fairfield Medical Center or Fairfield Healthcare Professional Location.

Yes. My FMC Portal has security measures within it that prevent unauthorized users from reading communications about your health. This information is accessible to read through the correct username and password to log in to your portal account.

Yes. Frank lin Medical Center’s patient portal is a secure website. All information is encrypted, and you will be the only one who can view your health information. You will be required to setup a username and password when registering for access to the patient portal.

Can you bring essentials to the hospital?from adventhealth.com

When visiting us, we recommend only bringing a few essential items with you. We are not responsible for any lost items, per our consent form. We suggest that any items that are brought with you to the hospital are sent home with your family or visitors.
