Through our patient portal you may securely access your personal health records. You may be able to view and manage changes to your demographic information (address, phone numbers, etc.). You will also be able to view your appointments, clinical summaries**, statements, and other documents.

How to contact Urban Health Plan?

If you need medical care after hours, UHP provides 24-hour, on-call service on weekdays, weekends and holidays. To reach a provider, please refer to the “Our Sites” tab on the web site and find the phone number for your health center. Give the operator your name and tell them that you are a patient at Urban Health Plan. The operator will contact the provider on call who will return your call as soon as possible. If the line is busy, please call (212) 517-1891. If you have a life-threatening emergency, call 911 for an ambulance.

Where does Medent Patient Portal come from?

Your Medent Patient Portal information comes directly from your electronic medical record at your doctor’s office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.

Who owns Medent Patient Portal?

Medent Patient Portal is owned and operated by Medent Patient Portal and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by Medent Patient Portal.

What is the Medent Patient Portal activation code?

Patients who wish to participate will be issued a Medent Patient Portal activation code during their clinic visit. This code will enable you to login and create your own user ID and password. If you were not issued an activation code, you may call your primary care clinic to get one or ask to sign up during you next office visit.

How long does it take to get a Medent patient portal answer?

You will generally receive an answer within 1-3 business days. Please note that Medent Patient Portal should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.
