Located in the McLeod Tyler Wellness Center, the Student Health Center provides a full-range of primary care services including the evaluation, treatment and prevention of all kinds of acute or chronic physical, mental and social health issues.

You can use the portal to perform a variety of actions including but not limited to: Upload your COVID vaccination record including booster shot and enter the vaccination date to the Student Health portal if you received your vaccinations at a retail pharmacy, clinic or provider other than SHC.

How do I access wellness portal for virtual visits?

For virtual visits, students can check in to their appointments and connect to Zoom for their virtual counseling or psychiatry sessions through the my.WellnessPortal. What is UChicago Student Wellness?

UChicago Student Wellness strives to make it convenient for students to make appointments, communicate with clinicians, and access health records. We have one integrated health record system for all your activity with UChicago Student Wellness: my.WellnessPortal. This integrated platform allows us to facilitate comprehensive, holistic care, and will enable many convenient features including online appointment scheduling, single-sign-on using your CNetID, and secure messaging with your clinicians. All registered students already have an account.

What is secure messaging?

The portal’s secure messaging feature provides students with a HIPAA-compliant means of communicating with providers that will protect your privacy. To message a counselor, psychiatrist, or provider, you must have seen that staff member before you can initiate a secure message. Please do not send secure messages for urgent matters.

Can students check in to Zoom?

What is WH&CS portal?

The WH&CS Patient Portal offers a variety of online options in an effort to improve patient access to SHC services; streamline your patient experience at SHC and facilitate communications between you and your providers and other SHC staff. The portal is a secure and confidential method to communicate regarding your non-urgent medical and mental health needs. You can use the portal to perform a variety of actions including but not limited to: 1 Follow up on your visit by communicating with your provider 2 Complete your TB High Risk Screening Questionnaire (new, incoming student requirement) 3 Enter your immunization information and upload immunization records (new, incoming student requirement) 4 View and print your immunizations 5 Request a prescription refill 6 Complete pre-appointment visit questionnaires and/or medical history forms 7 Complete required compliance forms prior to your appointment 8 Obtain your lab results 9 Schedule, view or cancel appointments online 10 View and print billing statements 11 Request a medical records release for an outside provider 12 Request a copy of your medical records 13 Ask a question about your insurance referral 14 Ask a question about admission health requirements

In an intiative that encourages UAB students to receive their COVID-19 vaccinations, UAB Student Health Services will be giving away "Vaxxed and Back at UAB" t-shirts.

The Patient Portal will give you access to online services such as: 1 Make an appointment (not available at this time) 2 Cancel an appointment 3 Upload Immunization Records 4 Complete Health History Questionnaire 5 Complete Meningitis Response Form 6 Submit Health Insurance Information 7 View educational material on a variety of subjects 8 Receive a Secure Message (an encrypted email) from your medical provider 9 Print Immunization Record 10 Opt In/Opt Out to text messages from the Health Center

To access the Patient Portal click on the Health Center Patient Portal link which can be found on your myoneonta or under Key Links on the left of this page . Once you click the link this page will display. Please click on Register at the top right of the screen.
