Parents and caregivers of younger patients should contact our Health Information Management team at 717-531-1697 to sign up for portal access (8 a.m. to 4:30 p.m. weekdays).

How to create a Hershey account?

You can create your account online using the Self-Enroll process. Or you may sign up with the office staff during any visit to the Milton S. Hershey Medical Center or one of our Medical Group locations.

How to join the Health Portal?

If you registered during your office visit, you should receive an email "invitation" to join the health portal. The link in this email message will take you to your registration information. If you do not see the message in your email inbox, you may want to check the junk or spam folder to see if it is there.

Does Penn State Hershey have secure messaging?

Only care providers who are part of the Penn State Hershey system will be able to access secure messaging to communicate directly with you.

Can parents get a copy of medical records from Penn State?

As always, parents and guardians can contact their family physician to discuss their child's medical situation, and can contact Penn State Hershey Medical Records office to obtain paper copies of medical records by providing proper identification.

When can a child access their medical records?

On the child's 18th birthday , parental or guardian access to online medical records will be terminated. The child will be able to initiate their own account for access to their electronic health record during their next office visit. Federal HIPAA law requires privacy of health records at age 18.

What is the purpose of Dr. Azar's book?

Azar has written this book to educate every woman about her bladder, bowel, and female anatomy in an interesting and understandable way. She has also explains why these parts may not work or why they give patients problematic symptoms. Everything from problematic periods and sexual function to infections and problematic bowel/bladder function is discussed. Dr Azar's hope is that every woman who reads this will understand and respect her pelvic parts and how they work enough to take care of them and to be able to troubleshoot health issues related to this important area of her body.

What is Azar's job?

Azar is often faced with solving complex problems in female health that involve detailed evaluations of patients' histories of multiple symptoms, health issues, and prior procedures/surgeries and she uses her extensive experience and expertise in addressing these more complex problems.

What is the number to call for a respiratory exam?

Hours or services may vary. If you are experiencing acute respiratory symptoms- fever of 100 or above, shortness of breath, coughing- you must call us at 423-837-5801 before your scheduled appointment.

Who is Kristin Flake?

Kristin Flake is a Family Nurse Practitioner and is board certified by the American Nurses Credentialing Center. She graduated from the University of Alabama in Huntsville with her Bachelor and Master of Science in Nursing degrees and has worked with Dr. Azar since 2004. She is licensed both as a Registered Nurse and an Advanced Practice Nurse in the state of Tennessee. Prior to completing her Master's Degree, she worked with Dr. Azar as a Registered Nurse. Kristin has completed a specialized training course in Colposcopy for Advanced Practice at Emory University Regional Training Center and has also been trained on insertion and removals of Nexplanon and Intrauterine Contraceptive devices. She has completed specialty courses in botox, dermal fillers, sclerotherapy, and weight management.

Who is Rena Azar?

Dr. Rena Azar began to practice gynecology in 1997. She is the only gynecologist in the Chattanooga area who is Board Certified in the subspecialty of Female Pelvic Medicine and Reconstructive Surgery. As a specialist, Dr. Azar has extensive experience in not only the health of the reproductive tract, but also of bladder and bowel function. She has over 20 years of experience in private practice and in the operating room here in Chattanooga. Dr. Azar is often faced with solving complex problems in female health that involve detailed evaluations of patients' histories of multiple symptoms, health issues, and prior procedures/surgeries and she uses her extensive experience and expertise in addressing these more complex problems.
