The Patient Portal is part of your personal electronic health record, and allows patients to conveniently access their health information from the comfort of their home or office.

Premier Medical Physicians is a group of experienced internal medicine and primary care providers who care for patients of all ages. We offer physicals, general wellness services, immunizations, pre-operative screenings, and disease management for chronic conditions such as high blood pressure and diabetes.

Since 2008, we have provided skilled, professional home health to thousands of patients in St. Louis, St. Charles, and surrounding areas. Using a secure password, patients can log into the online Patient Portal 24 hours a day, 7 days a week to: Download copies of medication lists and patient medication education literature Update demographic, medical and surgical history and allergy and medication information

Does Premiere respond to phone calls?
Many agencies do not respond to your phone calls. At Premiere, the Administrator and CEO are personally available to answer your questions and concerns.

If you are a patient who has not previously had a mystlukes portal account and you want to learn how to set up your account.
Self-Enrollment
If you are over the age of 18 and your email address has previously been provided to us, and is stored in our system, or if you have a medical record number (MRN) from our new EMR system (after 10/01/2018).
