What is medication reconciliation at Penn Highlands?

Penn Highlands DuBois is implementing a dedicated medication reconciliation team to work on completing patient’s medication histories. It is important for patient’s to be sustained on their home medications while they are inpatient to prevent any lapse in treatment. The team is made up of a pharmacist and three pharmacy technicians. We are located outside of the emergency room and are working on completing medication histories on patients that are targeted for admission. We will be discussing patient’s home medications with them while they are in the emergency department to create a “best possible” medication list. This list will follow them through their admission and be updated upon discharge with any new medications that are prescribed while being an inpatient.#N#We are currently available Monday thru Friday from 8:00 AM - 4:30 PM with plans to increase availability forthcoming.
