An authorization must be completed in its entirety to receive a copy of your medical records. Please mail the authorization back to the HIM Department at the appropriate address listed above. It does not reproduce a patient's entire medical record online, but instead provides access to selected information such as prescriptions, basic laboratory results, summary medical history, and appointments. UnityPoint Health reserves the right to limit health information in the patient portal on a case-by-case basis. For a copy of your full medical record, please contact the Health Information Management department of your hospital or the clinic administrator of your local clinic.

The purpose of this portal is to provide our patients with direct access to information about their health care treatment at UnityPoint Health and to communicate with their UnityPoint Health providers .
