What is follow my health?
Welcome to Follow My Health, your free, secure, personal gateway to your outpatient health information. Our online portal allows you to view your electronic medical record from the comfort of your home or office at your convenience.
What to do if you feel there is incorrect information in your medical record?
If you feel there is incorrect information in your medical record, you can call the TMH Privacy Officer at 850-431-5339. Another option is to download and complete the Request for Amendment/Correction of Health Information Form to request a change in your health information.
How to receive follow my health portal invitation?
To receive an invitation for the Follow My Health patient portal please call your physician's office to have one sent to you.
How long does it take for a lab to show up on a portal?
It can take up to 72 hours for lab results to show in your portal account. Other information should show the same day. You will not be able to see radiology results, such as x-rays and MRIs. For a complete copy of your medical record please contact your physician’s office and ask for medical records.
Where is my FMH health information?
Login to your FMH portal. Some of your recent health information will be at the top of the screen under the heading “Health Summary.”
Does TMH include hospital records?
Your information is obtained from your TMH Physician Partners outpatient electronic medical record. It does not include hospital records.

Does FollowMyHealth have two factor authentication?
When creating your account, you will have the opportunity to set up two-factor authentication and enter security information that only you will know ; for example, you might select a PIN sent to your mobile phone or the answer to a personal question.
Constantly crashes, worthless customer support
When I first got this app a couple of years ago, it worked great. Sometime last year, it suddenly started crashing as soon as I logged in. I could see my information momentarily, and then it would just close. I reinstalled it twice, I installed updates, nothing helped. So I contacted customer service.
Not very good
If this app worked consistently and the way it is supposed to work it would be amazing, but I have had many problems with this app. After having major heart surgery, I downloaded it only for the convenience of being able to “easily” pay my bills.
WORST APP EVER!!
Does not work at all!! Scheduled 3 appointments and it never showed up on my upcoming appointments tab. This app worked great 1 year ago..
