What is the Phelps Memorial portal?

Our portal provides easy, secure access to health information and online communication with our Team. All information is pulled from your Phelps Memorial electronic health record. To access your information, you must first create an account.

How do I set up a Phelps health account in MyChart?

If you have used previous Phelps Health patient portals (Passport, healow, eCW, etc.) you must register as a New User in order to set up an account in MyChart. Some of your information (such as allergies, medications and immunizations) from these other patient portals will be transferred over to your MyChart account.

How do I access my Phelps Memorial electronic health record (EHR)?

All information is pulled from your Phelps Memorial electronic health record. To access your information, you must first create an account. Once you have created your account, you will be able view your electronic health record by desktop computer , IOS application and Android application.

Patient portal

MyPhelps is a secure website that offers you access to your hospital records, medical history and other data. You can also use the MyPhelps patient portal to keep track of your dependents’ information.

Medical records

As a patient, you have the right to access your medical records. Copies of medical records are available to patients and physicians with the proper authorization. Our Health Information Management Office can help—call (914) 366-3060.

Support services

Phelps provides free interpreting services for patients who require assistance communicating. Notify your nurse if you need an interpreter.

Patient satisfaction

Offering you the best possible health care is our top priority. To determine where improvements are needed, we participate in the Hospital Consumer Assessment of Health Providers and Systems (HCAHPS) survey, a standardized tool for measuring and reporting satisfaction across all hospitals in the U.S.

Billing & insurance

Your hospital bill covers the cost of your room, meals, 24-hour nursing care, laboratory work, tests, medication, therapy and the services of hospital employees. You will receive a separate bill from your physicians for their professional services.

Secure online access to your accounts

MyPhelps is a completely secure website that allows you to have access to your hospital records, medical history and other data. You can also use the MyPhelps patient portal to keep track of your dependent’s information. Through MyPhelps, you can have complete control over your medical information.

Signing up for MyPhelps is easy

A representative from the Admitting team will give you a registration code when you arrive at the hospital for any service. For security reasons, this code will expire after seven days. You may use it to register for a MyPhelps account .

Printed medical records

If you prefer not to use the MyPhelps patient portal, the Medical Records Department (also called the Health Information Management [HIM] department) can also release your medical records to you. You may visit the HIM department at the hospital or request the information by mail or fax. Requests will be processed within 10 business days.

Where to mail a release of information form?

Fill out our Release of Information (ROI) form as a PDF or Word document. You can mail your complete and signed release form to 733 Cedar Street, Garberville, CA or fax it to us at 707-923-4433. If you need help filling out the form, download the ROI form instructions.

Do you need permission to release medical records?

To release medical records, we need to have your written permission .
