Use the MyChart patient portal to keep up-to-date with your health information, immunizations, allergies and medications. With MyChart, you also have access to request a prescription refill for a current medication, view your conditions and your plan for care, add/ remove medications and allergies.

Making an appointment at a clinic is now easier than ever. Use the MyChart patient portal to schedule an appointment, request an appointment or cancel an upcoming visit with your primary care physician. Prior to your scheduled visit with your provider, you can use eCheck- In to update insurance information, medications, allergies, fill out questionnaires and sign documents.

How to get a medical record number for labor?

Pre-register for labor and delivery with expected delivery date and location. If you have not signed up for MyChart, call 501-202-2299 to get a medical records number (MRN) created for you.

Can you send a letter to your primary care provider?

You can send a message to your primary care provider if any follow up questions arise regarding a current health concern. You will also be able to receive letters when needed, including school excuses, work excuses or referral letters.

How to download Baptist health records?

You can also download a copy of your My Baptist Health records at any time in either an Adobe® PDF or XML file format. Simply click on the “Download My Data” icon on the My Baptist Health home page, which will take you to the “More Options” page. Select the “Upload/Download Health Data” link. Your health record will automatically be displayed on the “Upload/Download CCD Format Health Data” page. Use the dropdown menu at the bottom of that page to select either PDF or XML. Then click the blue “Download My Data” icon. If required by your browser, click “Yes” to view the exported file.

Who can request access to their spouse's health information?

Legal guardians may request access to their ward's health information. Husbands and wives may request access to their spouse's health information. Adults may request access to their elderly parent's health information — with the patient's permission of course. Request Proxy Access.

Is Concord Hospital a separate portal?

The patient portal for Concord Hospital - Laconia and Concord Hospital - Franklin is separate from My Patient Connect. In the future, the two portals will be combined but for now, patients need to log in to separate portals based on where they receive their care.
