Call 801-393-5355 and ask our patient services representative to enable your Midtown Clinic Patient Portal. You will be asked to verify your identity and provide a valid email address. You will receive an email with a temporary password and activation link.

Midtown Medical Associates is a member of a network of physician practices that utilizes MyHealthHome, a secure, web based system that allows you real-time access to your medical record and enables you to contact our office electronically. MyHealthHome is available 24 hours a day for you to review your health records and conduct non-urgent communications with our office.

All messages sent to your provider’s office are transferred via a secure server compliant with HIPAA standards.

Is RWJBarnabas Health a centralized portal?

RWJBarnabas Health is in the process transitioning all of our hospitals to a centralized patient portal called MyChart where all hospitals’ records will eventually be contained within one system for your convenience. Please be assured that you will always have access to your medical records.
