When will I receive a copy of my medical record?from mercy.com

Please allow 30 business days for us to process your request. In some cases, the information you need may be stored at one of our offsite storage locations so additional time may be required to process your request. Please be sure to fill out the authorization form accurately. Inaccurate information on the form may cause delays in providing you with the information you requested.

Where is the FMC portal located?

If you do not have a scheduled appointment and wish to sign up for My FMC Portal, you need to go to the FMC Medical Records Department, which is located on the first floor at 401 N. Ewing St. in Lancaster, Ohio. The hours are 7 a.m.-8:30 p.m.

How old do you have to be to have a portal account?

Patients 13-17 may have their own portal account. Patients age 18 and older may grant proxy access to their portal account through an approved Personal Representative Authorization Form, which may be done at any Fairfield Medical Center or Fairfield Healthcare Professional Location.

Does FMC have a password?

Yes. My FMC Portal has security measures within it that prevent unauthorized users from reading communications about your health. This information is accessible to read through the correct username and password to log in to your portal account.
