Can a patient create a family login?from amitahealth.org

Yes. Patients can create a family login account that will allow them to access selected family members' health information within the same login account. Family members will have the same access to patient information and actions. In order to setup an account of this nature, log into each patient's portal account.

What is the phone number for a patient portal?

For problems logging in to your patient portal account, call 877-621-8014. If they are unable to help please call 231-935-2026 and select the option for the community where your care took place.

How old do you have to be to have a patient portal?

Any person age 13 and older with an active email address can have a patient portal. If you are the parent or legal guardian of a child under age 13, their account must be initiated by you. (See "What is proxy access?" below.)

How long can a parent access a child's health information?

Proxy access for patients 0-12 will be granted until the child turns 18 years old. Parent access will be limited automatically when the child turns 13.

What tests do not populate in the portal?

Some sensitive lab tests that do not populate into the portal are STD/HIV, toxicology, tumor markers, and blood typing.
