How to schedule a telemedicine visit?

At your appointed visit time, you will go to our website (here) and click on your provider’s name on the left hand side of the page (look to the left of this page), which will connect the telemedicine visit on a HIPAA compliant platform, Doxy.me. Expect to meet with your provider in real time and know that your information will be just as confidential as if you were in the patient room.

Does MFM have telehealth?

MFM has open patient slots available for patient care every day for in-office and telehealth visits. We are not seeing COVID-19 patients or any patient with symptoms vaguely similar to COVID-19 (or have risk factors for exposure) in the office. We see and manage COVID-19 patients and those at risk with telehealth. For nearly all insurances we can offer telehealth visits. If a visit requires an in-office visit we have refined our processes to minimize exposure.

What is a PCP in medical?

Your primary care physician (PCP) serves as your advocate and guide throughout your health care journey. If necessary, your Meritas Health North Kansas City internal medicine doctor can admit you to North Kansas City Hospital and will treat you during your stay, which means you’ll receive care in the hospital from a doctor you already know and trust.

What is the phone number for a pharmacy?

If you need urgent medical care after business hours, call 816-421-4240 to reach the on-call doctor. Medication refills will not be called in to the pharmacy after hours or on weekends.

As a parent or guardian of a child under age, 18, you may request proxy access to your child's records. You may request this access during a clinic visit. Or, you can complete and return MyChart Proxy Access Request - for Access to a Minor Patient's MyChart and receive a special access code to sign up as a proxy.

What is PCMH in family medicine?

What is a PCMH? PCMH stands for Patient-Centered Medical Home. Patient-centered medical care involves a team approach for every staff member to assist our patients in getting the medical care they need, including prevention and wellness, acute care, and chronic care. While Associates in Family Medicine has always worked toward patient centered care, we are currently redesigning our practice to improve the quality, effectiveness, and efficiency of the patient care we deliver while responding to each patient’s unique needs and preferences.

Is there a fee for attending a group class at Associates in Family Medicine?

These classes are provided by our medical staff for our established patients to learn more about certain topics. We are currently developing Group Classes for Smoking Cessation, Diabetes Management, Nutrition, Living Well and Chronic Pain Self-Management.
