What is the updox patient portal?
Kachina Family Medicine is excited to offer the Updox Patient Portal. Updox is a secure, password-protected portal that enables patients to access their medical records anytime and anywhere they have internet access. If you are an existing patient not currently enrolled in the patient portal and would like to be added, please contact our office.
How do I start using the FPC patient portal?
To start using the FPC patient portal, you will need an activation code from your FPC office. Please let us know if you need assistance.
How old do you have to be to communicate with FPC?
Parents/guardians may communicate with FPC providers and staff as you always have until your child reaches age 18.
Can a parent/guardian be a portal authorized representative?
For this reason, we find it necessary to ask our patients in this age group to complete a consent form naming a parent/guardian as their portal authorized representative. How to enroll in the patient portal?
To enroll in the patient portal, you must receive services at one of our locations. To create an account, you may self-enroll here. Alternatively, you may contact the location where you received care and tell them you would like to make an account. Registration requires an email and a valid ID.
How are messages sent to your provider's office transferred?
All messages sent to your provider’s office are transferred via a secure server compliant with HIPAA standards.
Is RWJBarnabas Health a centralized portal?
RWJBarnabas Health is in the process transitioning all of our hospitals to a centralized patient portal called MyChart where all hospitals’ records will eventually be contained within one system for your convenience. Please be assured that you will always have access to your medical records.
