New Cerner Patient Portal

Our upgraded EHR solution utilizes one platform for all patient records. This means that whether you are seen in our ER, have a wellness visit at a clinic or come in for an x-ray, all of your information is housed in one system which you will access through one patient portal.

Portal Features

Patients of Poche Medical Clinic, Lutcher Family Clinic, St. James West Bank Clinic and St. James Urology Clinic can book appointments through the portal. Simply log in and click “Schedule an Appointment.” Choose the type of visit (“established” means you have seen a provider before) and from here, view openings and choose an option.

Past Portals & Bills

Before our new Cerner System, patients had two separate portals to access information from the hospital and clinics. If you were previously signed up on our past portals, you can still access them for health information and bills prior to August 23rd (see links below).

What is JPS connection?

JPS Connection is our county assistance program that helps eligible Tarrant County residents by providing quality healthcare at an affordable cost. The program does require a completed application and supporting documents to determine eligibility. Once eligible, coverage is provided and can only be used at JPS facilities.

How many visitors per day in a hospital?

Updated visitation guidelines can be found Here. Most inpatients will be allowed one visitor per day.

Do JPS patients have to wear masks?

All JPS team members, patients, and visitors are required to wear a mask at all times when on JPS property, regardless of vaccine status. There are no changes to our mask policy at this time. Thank you for your cooperation as we continue to ensure the health and safety of our community.

Do you need a mask in JPS?

There are no changes to the current PPE practices. Masks are required for anyone inside JPS facilities.

Phoebe Patient is the quickest and easiest way for you to access your medical records from your computer or cell phone. You can also email [email protected] to request your medical records. Please include a call back telephone number, patient’s full name, date of birth, the visit date of the records needed, and the address where the medical records can be mailed or if pick-up is preferred. All requests will be processed within 3 business days or patient will be notified of any delay.

Patients who have used any hospital within Phoebe Putney Health System (PPHS) and are 18 years of age or older. Parents can complete a Child Proxy request and have access to their minor children’s information. Patients can also give another adult access by completing the Adult Proxy request and guardians/healthcare agents who have a Durable Power of Attorney can request access by completing the Adult Proxy with DPOA request.

Can you bring a copy of a picture ID to the Health Information Department?

Additionally, patients may come to the Health Information Department to request records in person. Please remember to bring a copy of a picture ID. While we strive to provide records at the time of request that is not always possible.

This form is completed by a parent/guardian who wants access to PHI for child under 18.
