What is a patient portal?

A patient portal is a website for your personal health care. The online tool helps you to keep track of your health care provider visits, test results, billing, prescriptions, and so on.

What information is available in the och patient portal?

The OCH Patient Portal gives patients direct access to medical information including; Allergies, Immunizations, Active and inactive medications, Current and past health concerns, and test results. A limited number of same-day test and procedure results will be available online

What are the patient portal policies and limitations?

The patient portal is provided as a courtesy to our valued patients. We are focused on providing the highest level of service and health care. However, we have the following policies and limitations: Do not use the portal communication if there is an emergency, please dial 911 or go to the nearest Emergency Department.

What is the Patient Portal?

The Patient Portal is not intended to help diagnose a medical condition, provide any sort of treatment advice, or emergency help. If you are experiencing a medical emergency, you should be seen by the Emergency Department or you should call 911.

When will Princeton Community Hospital open EHR?

Beginning July 1, 2019, both Princeton Community Hospital and Mercer Medical Group Affiliate patients may access their medical record through the Patient Portal thanks to EXPANSE, our new electronic health record (EHR) designed to create a more seamless, efficient, and unified experience for the patient.

What is proxy access?

A proxy access is granted for three patient types: 1). Parents or legal guardians of minors up to the age of 12. 2). Parents or legal guardians for patients with mental or physical impairments. 3). Adult to adult patients.

What is the phone number for HIM?

If you did not provide your e-mail address while being registered, please call HIM department, 304-487-7257, Monday-Friday. 8:00 a.m. – 4:30 p.m. and provide your e-mail address. Your e-mail address will be added to your account information. Once this information is added, you can self-enroll at your convenience.

Is proxy access protected by HIPAA?

The person authorized to have proxy access to your records is not covered under HIPAA and is not subject to federal health information privacy laws. If that person intentionally or unintentionally discloses health information about you, that information might no longer be protected by privacy regulations.

Can a patient access their medical records?

Patients and/or legal guardians accessing their records through the Patient Portal may view, print, and/or forward their medical record information. The Patient Portal will be available 24/7 after the patient has registered to use the site. Patients may view their own records, and they may also view the medical records of a family member if the family member has given them permission to do so. This is called “Proxy Rights.”

How do I use a Patient Portal?

If your provider offers a patient portal, you will need a computer and internet connection to use it. Follow the instructions to register for an account. Once you are in your patient portal, you can click the links to perform basic tasks. You can also communicate with your provider's office in the message center.

What are the benefits of patient portal?

The Benefits of a Patient Portal. You can access your secure personal health information and be in touch with your provider's office 24 hours a day. You do not need to wait for office hours or returned phone calls to have basic issues resolved.

How old do you have to be to access a child's patient portal?

If you have a child under age 18 years , you may be given access to your child's patient portal, too. Providers can also get in touch with you through the portal. You may receive reminders and alerts. You will receive an email asking you to log in to your patient portal for a message.

How much does an e-visit cost?

For minor issues, such as a small wound or rash, you can get diagnosis and treatment options online. This saves you a trip to the provider's office. E-visits cost around $30.

Can you access all of your health information?

You can access all of your personal health information from all of your providers in one place. If you have a team of providers, or see specialists regularly, they can all post results and reminders in a portal. Providers can see what other treatments and advice you are getting. This can lead to better care and better management of your medicines.

What to do if you have problems with portal?

If you are experiencing problems with the portal, please call your provider’s office.

How to find an appointment form?

You can find a Request an Appointment form using the Appointments tab in Menu. Once the form is completed, a member of your provider’s office will reach out to confirm. This is for non-urgent issues only.

How old do you have to be to get proxy access?

Proxy access can be granted for children up to age 12. No proxy access will be given for patients ages 13-17 years. Prior to the minor's 13th birthday, you will receive a notification email stating that the account will be disabled.

How to pay my adobe bill?

Please call (866) 853-9551 to pay your bill. Or, click on the "Pay Your Bill" at the top of the webpage and you can pay your bill online.

How to send health information to another practice?

You are able to send health information you choose to another practice by choosing the “Chart” button in Menu, selecting the information you would like to send, then at the bottom of the page selecting the “Send to Another Practice” button.

Can you take medication through the patient portal?

Yes, through the medications option in the patient portal menu bar.
