Who is Horizon behavioral health?

Established in 2011, Horizon Behavioral Health (HBH) is a private multidisciplinary practice providing psychiatric, psychological and therapy services. HBH has two offices: Hinesville, GA, and Rincon, GA. With nine providers and fifteen administrative staff members, we approach each patient with a team effort of highly skilled professionals.

Where can I find more health information about Horizon Health?

What is patient portal?

The Patient Portal is an online service that provides patients secure access to their health information. Various features may be available on the portal at your practice's discretion, including the ability to send messages to your health care providers, schedule appointments, and pay bills online.

How to reset password on patient portal?

Sign in to the Patient Portal, go to Security Settings, and reset your password. Click Forgot your password on the sign-in page and enter your email address to request a password reset email. Contact your provider's office and request a password reset email.

Can you delete messages from the Patient Portal?

You cannot permanently delete sent or archived messages . This is because all messages that you send and receive in the Patient Portal are part of your medical record.

Can you make your test results available on the patient portal?

It is at your provider's discretion to make test results available. Your provider must authorize the release of your test results in order for them to post to your Patient Portal account. Only test results which are considered appropriate for release will be accessible through the Patient Portal.

What is a client referred to as in Horizon?

Clients are referred to PACT by Horizon’s Access team or a case manager. Other clients join PACT while transitioning from state hospitals or jails.

How is a substance use disorder treatment track determined?

Specialized Substance Use Disorder treatment track is determined based on an initial and ongoing assessment to determine the level of care an individual needs. Based on level of care services may include a combination of: Intensive Outpatient (three meetings per week for three hours), Outpatient Group (1 hour per week), and Individual and Family Treatment. Within this programming a specialized women’s program is available. Ancillary services include: Motivational enhancement, early recovery and relapse prevention skills, family education and support for ongoing recovery. Clinicians who provide this service are Licensed, Licensed Eligible, or Certified Substance Use Clinicians.

What is an outpatient service?

Outpatient services are designed to meet behavioral and emotional concerns that individuals experience throughout their lifetime.

How old do you have to be to be admitted to a mental hospital?

Criteria for Admission: The person must be 18 year old or older at high risk for psychiatric hospitalization or for becoming or remaining homeless due to mental illness. The client might also require intervention by the mental health or criminal justice system due to inappropriate social behavior.

Can you make an admission appointment through telehealth?

ALERT: During the COVID-19 public health crisis, if you need to make your first appointment, you can schedule an Admissions appointment through Telehealth, however we are still accepting walk-ins at this time. All current consumers also have the opportunity to see their Horizon providers through Telehealth.
