What is an elevelevate health?

Elevate Health is a different kind of medical office. We are a board-certified primary care medical doctor and a variety of complementary healthcare providers who collaborate all under one roof. Our goal? To provide medical care with a holistic approach to health optimization.

What services are covered by elevate health?

This plan covers primary care, functional and family medicine, nutrition counseling and health coaching. As a member, you experience personalized care and 24/7 access to your primary care team. We also offer the following services independent of membership: You are something special. And so are we. Elevate Health.

Who is elevatepfs?

At ElevatePFS, we have industry experts located from coast to coast with state-specific expertise in all 50 states, and more than 1,500 client facilities nationwide. Elevate Patient Financial Solutions SM provides an innovative approach to patient financial advocacy and revenue recovery.

What is Elevate Health Group?

Elevate Health Group is founded on the principles of a traditional Internal Medicine practice. Our patients' care will be continuous by their regular physician, whether in the outpatient setting, hospital setting, or in need of a skilled nursing facility. Our physicians value continuity of care and hold the belief of it to be the basis of effective medical practice.

Where is Elevate Health Group located?

Elevate Health Group officially opened its doors in August of 2015. Our brand new state of the art office is located in Downtown Glendale. Our aim is to provide our patients with the highest level of care, with an evidence-based and modern approach.

How many doctors are in Elevate Health Group?

Our group is currently comprised of six physicians and a nurse practitioner. Elevate Health Group provides a wide array of services including Annual Physicals, Chronic and Complex Medical Management, In-patient Hospital Services, Nursing Home Visits, Well-Visits, Pre-Operative Medical Optimization, Sports Physicals, STD Screenings, Vaccinations, and much more.

In the Event of an Emergency

Please all 911 or go directly to the emergency room. Elevate Health does not offer a paging service and doctors will not reliably answer patient messages after business hours. Any after-hours communication may incur a charge.

Need to schedule a visit?

Call or text us at 406-219-3631 and one of our Patient Care Coordinators will get you scheduled with your doctor.

Call or Text the Office

The best way to get a time-sensitive message to your doctor during business hours is always to call the office. If you contact us outside of business hours or during a holiday closure, we will be in touch the next business day.

Message through the Elation Passport

If you would like to send an email to your doctor you can message them directly through the Elation Passport. This is a secure portal where you can directly message your doctor and access your health records. The Elation Passport can be used in a web browser or through the mobile app .

Direct Messaging

If you are a Primary Care Member, you can use Spruce Health, a mobil app that allows you to quickly message your doctor. When you start your membership, you will be invited to Spruce Health via SMS. Please contact the office if you need us to re-send your invitation.

Refill Policy

We have a standard 48 hour turn-around time for refilling all prescriptions. If you are out of refills, please contact your pharmacy directly and they will send us a refill request. Doing this promptly will shorten your refill time and ensure that you do not have a lapse in your medication.

In-Office Dispensary

We carry many of our most commonly used physician-grade supplements in the office, but if our stock is low or we do not carry a specific supplement that the doctor is recommending for you, we are happy to special order those products into the office for you.
