Patients who may qualify include those who: Live on the street or a place not meant to be a permanent home (tent, garage, storage unit, car, van, or trailer) When you arrive at Daily Planet Health Services as a first time patient you will be asked to fill out a Patient Registration/Financial Screening form.

When you arrive at Daily Planet Health Services as a first time patient you will be asked to fill out a Patient Registration/Financial Screening form. The cost for services is determined during the financial screening process. A sliding fee scale ( available here) is used for those who qualify.

Daily Planet Health Services’ mission is to provide accessible, comprehensive, and integrated quality health services to anyone regardless of their housing, financial or insurance status.
Payment
Payment is expected at time of service. If no payment is made, you may be rescheduled (not seen that day). Unpaid service fees will be billed to your account.
Registration and Renewals
New patients and patients returning after one year or more must first go through Daily Planet Health Services’ Central Registration before receiving services.

What is DPHS policy?
It is the policy of the Daily Planet Health Services (DPHS) to support and protect the fundamental human, civil, constitutional, and statutory rights of each person receiving its health care services. Furthermore, each patient shall be informed of his/her rights in the language that the client understands.
