Who is Cortland health center?

The Cortland Health Center was established in 1990, and was owned and operated as a private practice from 2001 to 2015, when we merged with Family Care Medical Group. As members of Family Care Medical Group, we are a part of a larger, physician owned and managed organization with sites in urban, rural and suburban Central New York.

Is there a nurse practitioner in Cortland County?

Cortland Health Center also has a Nurse Practitioner and a Physician Assistant that are also accepting new patients. ****In the event that there is a State of Emergency declared in Cortland County, the office will be closed****

What information can I find in the hospital patient portal?

At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records.

Financial Assistance

If you do not have health insurance or worry that you may not be able to pay for part or all of your care, we may be able to help. Guthrie provides financial aid to patients based on their income, assets, and financial needs. Learn more.

Interfaith Chapel

If you are in need of a private area for prayer, silence, or reflection, please visit our interfaith chapel. The chapel is located on the first floor near the Admitting/Registration reception area. Open between 5:30 a.m. to 11:00 p.m., it is a place that welcomes and offers solace to people of all faiths.

Interpreter

Sign language, oral interpreters and other auxiliary aids and services necessary for effective communication are available free of charge upon request to people who are deaf or hard of hearing. Translators for those who do not speak English are also available upon request.

How to enroll in the patient portal?

To enroll in the patient portal, you must receive services at one of our locations. To create an account, you may self-enroll here. Alternatively, you may contact the location where you received care and tell them you would like to make an account. Registration requires an email and a valid ID.

How are messages sent to your provider's office transferred?

All messages sent to your provider’s office are transferred via a secure server compliant with HIPAA standards.

Can you access read only medical records?

At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records. Due to the sensitive nature of radiology and pathology test results, these are not available on the patient portal and must be obtained through the hospital’s Health Information Department.

Is RWJBarnabas Health a centralized portal?

RWJBarnabas Health is in the process transitioning all of our hospitals to a centralized patient portal called MyChart where all hospitals’ records will eventually be contained within one system for your convenience. Please be assured that you will always have access to your medical records.
