When is full immunity required for CareSouth Carolina?from caresouth-carolina.com

In accordance with the CMS Omnibus COVID-19 Health Care Staff Vaccination Interim Final Rule and CareSouth Carolina policy, all employees must be fully immunized for COVID-19. Proof of full vaccination must be presented prior to beginning work with CareSouth Carolina. Beginning January 4, 2022, full immunity will be required before a new employee can begin work. Full immunity is when both doses of a two dose vaccine or one dose of a one dose vaccine is received + 14 days. Example: A 2 nd dose of the Moderna vaccine was received on January 3, 2022. Full immunity would be on January 17, 2022.

Care/case Management services will include but are not limited to referral appointment, answering questions from the patients regarding, screenings and preventive care, assistance with area agencies providing diet and nutrition services, questions about referral lab tests, setting and motivational interviewing for self-selection of self-management goals, and other case management services as needed . Will go on home visits when needed.

POSITION SUMMARY : The Clinical Patient Navigator-PrEP/HCV position will assist medical providers in the administration of patient care in assigned clinical areas within the scope of his/her training. Their primary role is to provide care/case management services in the support of patient care delivered inside and outside of our medical centers.
