Parents/guardians may communicate with FPC providers and staff as you always have until your child reaches age 18.

What age do you have to be to disclose medical information?

For Patients Who Are Minors: Due to federal and state confidentiality laws, patients who are between the ages of 14 and 17 are not required to disclose certain types of medical information to parents/legal guardians.

Can a parent/guardian be a portal authorized representative?

For this reason, we find it necessary to ask our patients in this age group to complete a consent form naming a parent/guardian as their portal authorized representative.
